Saberes y prácticas del profesional de enfermería en el paciente quemado

Abstract

RESUMEN: Introducción: Las quemaduras son un importante y creciente motivo de consulta y hospitalización, que causa en muchos pacientes grados variables de incapacidad y muerte. Además, es una situación traumática desde el punto de vista físico y emocional que tiene en la enfermera un partícipe en el cuidado de este tipo de pacientes. En el contexto colombiano se encuentran pocas investigaciones, en parte por las escasas unidades existentes, lo que motiva a indagar sobre los saberes y prácticas del profesional de enfermería en el cuidado. Objetivo: Comprender los saberes y prácticas del profesional de enfermería en el cuidado del paciente quemado. Metodología: Investigación cualitativa tipo estudio de casos. Participaron 10 enfermeras de las unidades de quemados del Hospital Universitario San Vicente Fundación HUSVF y 3 enfermeras de dos hospitales de IV nivel de complejidad del país, a quienes se les realizaron entrevistas semiestructuradas. Por otra parte, durante 71 horas de observación participante en HUSVF, se realizaron 6 entrevistas informales a dos cirujanos plásticos y cuatro auxiliares de enfermería. El análisis de los datos fue inductivo y concomitantemente se realizó triangulación hermenéutica de técnicas y datos dando lugar a la categorización emergente. Resultados: Emergieron tres categorías: trabajar en un contexto diferente, enfermera: eje central; y saberes y prácticas aprendidas. Conclusiones: El saber del cuidado del paciente quemado se obtiene a través de la experiencia y se perfecciona con el tiempo y la práctica. Además, se considera al profesional de enfermería como el eje central de la unidad de quemados tanto para el paciente, la familia y el equipo multidisciplinar. ABSTRACT: Introduction: Burned patients are an important and growing reason for consultation and hospitalization, causing in many patients varying degrees of disability and death. Being burned is a traumatic situation that a person can live physically and emotionally and the nurse participates in the care of this type of patients. Few investigations are found in the Colombian context, partly because of the few existent units, which motivates to inquire about the knowledge and practices of the nursing professional in this context of care. Objective: To understand the knowledge and practices of the nursing professional in the care of burned patients. Methodology: qualitative study type case study. Ten nurses from the burn units of the Hospital Universitario San Vicente Fundacion HUSVF and 3 nurses from two hospitals of the IV level of complexity of the country participated, to whom semi-structured interviews were conducted; In addition, during 71 hours of participant observation in HUSVF, 6 informal interviews were conducted with two plastic surgeons and four nursing assistants. The analysis of the data was inductive and concomitantly hermeneutical triangulation of techniques and data leading to the emerging categorization. Results: 3 categories emerged: work in a different context, nurse: central axis; and knowledge and practices learned. Conclusions: The knowledge of burned patient care is obtained through experience, it is perfected with time and practice. In addition, the nursing professional is considered as the central axis of the burn unit for both the patient, the family and the multidisciplinary team.
