Significado de calidad de la historia clínica : una mirada desde los médicos internistas

Abstract

RESUMEN: Objetivo: Comprender el significado que para los médicos internistas de Hospital Pablo Tobón Uribe de Medellín tiene la calidad de la información de la historia clínica. Materiales y Métodos: se realizó estudio cualitativo, basado en la metodología de teoría fundamentada, a partir de 15 entrevistas semiestructuradas a médicos internistas vinculados al Hospital Pablo Tobón Uribe: 10 individuales y una entrevista focal a 5 médicos. El análisis se hizo mediante la codificación de la información en forma interrelacionada desde cuatro momentos, en el siguiente orden: codificación abierta donde se conceptualizó, fragmentó y describió la información, luego se clasificaron por categorías descriptivas, para luego relacionar mediante la codificación axial las categorías a subcategorías por propiedades y dimensiones, realizando el respectivo análisis, dando lugar a las interpretaciones y obteniendo propiedades o atributos sobre los conceptos centrales. (Strauss & Corbin, 2002). Resultados: se evidencia el fenómeno de la disociación entre el deber ser y el ser del diligenciamiento con calidad de la historia clínica en los médicos internistas. Conclusiones: el médico debe ser consciente del valor inestimable de la información consignada en la historia clínica, no solo para las instituciones, el gobierno y, en general, para los diferentes actores del sistema de salud, sino primordialmente para orientar el accionar de los equipos de salud hacia la salvaguarda de la salud de los individuos y comunidades

ABSTRACT: Objective: To understand the meaning that the quality of the information in the medical record has for the internists of the Pablo Tobón Uribe Hospital (PTUH) in Medellín. Materials and Methods: A qualitative study was carried out, based on the grounded theory methodology, 15 semi-structured interviews with internists linked to the PTUH where performed: 10 of them individually as well as a focal interview with 5 doctors. The analysis was done by encoding interrelated information, in 4 moments, ordered as it follows: open coding of the information, where it was conceptualized, fragmented and described in detail. Then they were classified in descriptive categories, to finally connect all the categories and subcategories through axial coding by their properties and dimensions, allowing us to perform the respective analysis, obtaining the defined properties and attributes about the main concepts (Strauss & Corbin, 2002). Results: The evidence shows a disassociation phenomenon between the how it should be filled out a medical record and how is actually done to obtain quality in a medical record in general practitioners. Conclusion: Doctors must be aware of the invaluable worth of the information recorded in the medical record, not only for the medical institutions but for the government and, in general, for the different parts involved in the health system. But primarily, to guide the actions of the health teams, towards safeguarding the health of individuals and communities
