ABSTRACT: the aim of this study is to develop a state of the art on the influence of the social representations by the medical staff on the quality of care and adherence to treatment in HIV seropositive homosexual men, through the review of research articles from 2011 to 2021. To carry it out, a bibliographic search was conducted through a data collection plan that included the following stages: contextualization, classification, categorization, and results. Among the most relevant results, it is highlighted that despite scientific advances that have allowed a better understanding of the behavior of HIV and the social dynamics involved in living with the virus, social representations persist and are reinforced in the case of homosexual men, appearing in the form of stigma and discrimination against them. This discrimination has several negative consequences on the health of PLHIV and affects both initiation of and adherence to treatment. On the other hand, it became evident that discrimination against PLHIV in clinical settings is a reality; however, due to the effects of internalized stigma, some situations that are reported as discriminatory may not be so. Finally, there is a need to improve the services provided by the health system, including the specialization/training of personnel since, according to what was found, most of the barriers and discriminatory situations can be explained by shortcomings of the system or lack of training of the health staff
