Programa de telemedicina en epilepsia infantil en un hospital universitario: sistematizacion de experiencia

Abstract

RESUMEN: Introducción: A partir de la pandemia por COVID y las restricciones para las consultas presenciales en entornos ambulatorios, se aumentó el nivel de implementación y uso de la telemedicina en muchos hospitales a nivel mundial. En Colombia, el Hospital Universidad del Norte implementó desde inicios de 2020 el programa de tele neurología para dar seguimiento ambulatorio por telemedicina a pacientes con epilepsia en la infancia y adolescencia. Este estudio evaluó la experiencia del programa de tele neurología en pacientes pediátricos con epilepsia en un Hospital Universitario en Soledad -Atlántico durante la pandemia por COVID-19. Materiales y métodos: Estudio cualitativo de tipo sistematización de experiencias de diferentes actores del programa. Se realizaron entrevistas semiestructuradas, con posterior codificación, categorización y condensación de hallazgos en cuatro ejes temáticos 1 (dificultades, necesidades, logros y aprendizajes). Finalmente se interpretaron los resultados y se formularon recomendaciones y aprendizajes extraídos. Resultados: Se realizaron 7 entrevistas en total, y los hallazgos se condensaron en 3 necesidades, 4 dificultades, 4 aprendizajes y 3 logros. Es común a todos los participantes de la sistematización, la oportuna y eficiente implementación del servicio de telemedicina en tele epilepsia, y con esto haber logrado mantener unos esquemas de seguimiento a pacientes durante la pandemia. Adicionalmente, resalta la necesidad de alfabetización digital a los participantes, mejorar redes de conectividad y ajustar procesos administrativos acorde a las atenciones a distancia. Conclusiones: La experiencia de atención por telemedicina en pacientes con epilepsia infantil del Hospital Universidad del Norte fue exitosa, manteniendo un adecuado seguimiento de pacientes durante la pandemia, resaltando hallazgos importantes que están asociados al entrenamiento en telemedicina de los participantes, ajustes a los procesos de apoyo e identificación de potenciales barreras tecnológicas que afectaron la conectividad.

ABSTRACT: Since the COVID pandemic and the restrictions for face-to-face consultations in outpatient settings, the level of implementation and use of telemedicine has increased in many hospitals worldwide. In Colombia, the Universidad del Norte Hospital implemented the teleneurology program since the beginning of 2020 to provide outpatient follow-up by telemedicine to patients with epilepsy in childhood and adolescence. This study evaluated the experience of the teleneurology program in pediatric patients with epilepsy at a University Hospital in Soledad -Atlántico during the COVID-19 pandemic. Materials and methods: Qualitative study of the systematization type of experiences of different actors of the program. Semi-structured interviews were conducted, with subsequent coding, categorization and condensation of findings into four thematic axes (difficulties, needs, achievements and learning). Finally, the results were interpreted and recommendations and lessons learned were formulated. Results: A total of 7 interviews were conducted, and the findings were condensed into 3 needs, 4 difficulties, 4 learnings and 3 achievements. Common to all the participants in the systematization is the timely and efficient implementation of the telemedicine service in tele-epilepsy, and with this, having managed to maintain patient follow-up schemes during the pandemic. Additionally, it highlights the need for digital literacy for the participants, improving connectivity networks and adjusting administrative processes according to remote care. Conclusions: The experience of telemedicine care in patients with childhood epilepsy at Hospital Universidad del Norte was successful, maintaining an adequate follow-up of patients during the pandemic, highlighting important findings that are associated with the training in telemedicine of the participants, adjustments to the processes of support and identification of potential technological barriers that affected connectivity.
